Workflow Optimisation is a process designed to support seamless management of incoming correspondence, for our surgeries. This ensures patients are prioritised correctly and incoming and outgoing paperwork is dealt with efficiently to save clinical time and ensure the practice’s clinicians see the right people at the right time.
Children’s Health & Wellbeing Navigators work with and for GP practice teams who are trained to support children and their carers / families by signposting them to the most appropriate professional or service. This ensures that patients receive the right care, first time and as efficiently as possible.
This is designed to increase access to primary care services and incudes enabling appointments to be made available outside of normal practice hours between 6.30-8pm on weekdays, and Saturdays. The change means that some patients may go to places other than their usual surgery to be seen, but all sites are accessible and local for patients
First Contact Physiotherapists can help patients with musculoskeletal issues such as back, neck and joint pain by assessing and diagnosing issues, giving patients their expert advice on how best to manage their conditions. referring them onto specialist services if necessary.
The Care Home Team is a Multi-Disciplinary Team which has been set up under the NH|S England’s ‘Enhanced Health in Care Homes Framework’ (EHCH) to offer People living in care homes the same level of support as if they were living in their own home. The team achieve this through collaborative working between health, social care, Voluntary, Community, and Social Enterprise (VCSE) sector and care home partners The Enhanced Health in Care Homes (EHCH) model moves away from traditional reactive models of care delivery towards proactive care that is centred on the needs of individual residents, their families and care home staff.
Mental Health Workers are qualified mental health nurses who work with and for our GP practices. They work as part of the PCN Multidisciplinary team, provides a bridge between primary care and specialist mental health providers. They can draw on a range of provider mental health service and improve integration between primary care and mental health.
