PCN early adopters launch their integrated team

As with all PCNs we work with our partners to provide proactive, personalised care for people close to home. The East and West PCNs are led by a clinical director, who is a GP, supported by a team of highly qualified health professionals.
Total Health Excellence Primary Care Network (THE PCN) and Channel Health Alliance will be hosting a cancer awareness event in association with Macmillan Cancer Support on Thursday 15th August 2024 at Three Hills Sports Park in Folkestone, between 10AM and 2PM.
There will be a number of local charities and organisations in attendance, as well as free refreshments and children’s activities.
We hope to see you there!

Total Health Excellence East and West Primary Care Networks (PCN) are totally committed to having our partners, patients, their families, and members of the public involved in the decisions we make about the services we need to provide to a range of people in our areas.
Our PCN Public, Patient, and Partner Engagement group has been operational since September 2020 and meets every three months as a group.
It was agreed that it would replace the need for our member practices to have individual Practice engagement groups, saving duplication and valuable time spent within the practices.

• Lydden Surgery
• White Cliffs Medical Centre
• Guildhall Street Surgery
• The New Surgery
• Sandgate Rd Surgery
• Manor Clinic
• Pencester Surgery
The group is still recruiting new members. If you are interested in joining this group, please enquire via your GP practice.
