Services

Care Homes Team

The Total Health and Care Service is attached to the GP surgeries in our Primary Care Network. The service involves the coming together of all local health and social care organisations, to provide excellent care and support in patients’ own homes where possible, and in other places within the local area, when necessary.

The Total Health and Care Service has been developed based on an Integrated Neighbourhood Team (INT) model.  The development plans for this service which concluded towards the end of 2024, involved input from patient participants, members from local NHS organisations, social care, the voluntary and care sector and public health and has become a fully established and well recognised service in our area.

Unpaid carers, who care for relatives and friends in their own homes, often benefit from this system as much as the patients themselves – being able to access support,  which in turn prevents many health issues from arising because of their own stress and a lack of fully understanding their relatives’ and their own arising conditions.

Social Prescribers

Our Social Prescribers work with patients from our practices (in their own homes if necessary) to look at their individual needs and refer people to a range of local, non-clinical services which can help to improve their health and social needs  

Dieticians

Dieticians are experts in nutrition who help and advise people to improve their health and treat diseases or conditions, by education and giving practical, personalised advice to clients, patients, carers and their families.

Podiatrist

A podiatrist is an expert in foot care. Podiatrists help people in the care of their lower limbs including the foot and ankle and may also be involved in supporting older people to reduce their risk of falling. 

Clinical Pharmacists

Clinical pharmacists work as part of the general practice team to improve value and outcomes from medications.  They can consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and offer better access to health checks. 

Workflow Optimisation

Workflow Optimisation is a process designed to support seamless management of incoming correspondence, for our surgeries.  This  ensures patients are prioritised correctly and incoming and outgoing paperwork is dealt with efficiently to save clinical time and ensure the practice’s clinicians see the right people at the right time. 

Children's Health & Wellbeing

Children’s Health & Wellbeing Navigators work with and for GP practice teams who are trained to support children and their carers / families by signposting them to the most appropriate professional or service. This ensures that patients receive the right care, first time and as efficiently as possible. 

Enhanced Access Services

This is designed to increase access to primary care services and incudes enabling appointments to be made available outside of normal practice hours between 6.30-8pm on weekdays, and Saturdays. The change means that some patients may go to places other than their usual surgery to be seen, but all sites are accessible and local for patients  

First Contact Physiotherapist

First Contact Physiotherapists can help patients with musculoskeletal issues such as back, neck and joint pain by assessing and diagnosing issues, giving patients their expert advice on how best to manage their conditions. referring them onto specialist services if necessary. 

Mental Health Workers

Mental Health Workers are qualified mental health nurses who work with and for our GP practices. They work  as part of the PCN Multidisciplinary team, provides a bridge between primary care and specialist mental health providers. They can draw on a range of provider mental health service and improve integration between primary care and mental health.
