PCN early adopters launch their integrated team

The first of our four ‘early adopter’ Integrated Neighbourhood Teams, Total Health Excellence (THE) east and west, held their launch event at Hawkinge Community Centre on Thursday, 7 September 2023.

THE is starting a journey to deliver an ambitious vision for integrated working, building on a proactive care model, previously tested in the area, which proved to have positive outcomes for the local population.

The INT includes patient participation groups, NHS, social care, voluntary and care sector and public health. Clinical Lead Dr Tuan Nguyen said: “This is an exciting opportunity to work differently and improve services for the 80,000 people we serve. The focus of our INT will be multi-disciplinary team working (MDT)  and a shared approach to education and learning, especially for unpaid carers.”

The other PCN early adopters; Canterbury North and South, Mid Kent and The Marsh, will also be meeting shortly to discuss their priorities. Discussions are underway with the next cohort of fast followers, including PCNs in Thanet.

Dr Sadia Rashid said: “It was great to see such a broad range of partners taking valuable time to attend the Mid-Kent INT vision to address same day access for 0-16 year olds and to develop a same day access and proactive case management for the over 75 population. There was great enthusiasm and energy in the room and I am looking forward to seeing this programme of work develop.”
